Yes, but it’s important that you and your parent/guardian accurately complete the health history portion of the sports physical form prior to your appointment.
You can schedule an appointment any time during the year. Our appointments fill up quickly, so it is important to make an appointment at least 2 weeks before your tryouts or practices begin.
